Payments and Refunds

Accepted Forms of Payment

We accept cash, credit/debit cards, and FSA/HSA cards. Checks are accepted only for patients with an established history in our office. Returned checks are subject to a $50 service fee (or the maximum allowed by law). For qualifying treatment plans, we offer third-party financing options.

Insurance

We are out of network with all insurance plans. This allows us to focus on providing high-quality, health-centered care without insurance limitations. As a courtesy, we will submit claims on your behalf for traditional and PPO plans. We are unable to bill HMO or government-sponsored plans (such as Medicare/Medi-Cal/Denti-Cal). Insurance payments may be sent either to you or to our office and typically take a few weeks to process. Please note:

  • Insurance coverage and reimbursement are not guaranteed
  • Patients are ultimately responsible for all fees regardless of claim outcome
  • We are not responsible for claim denials due to plan limitations, eligibility issues, or processing errors

Payment Policy

A credit card on file is required to reserve appointment time, and payment in full is due prior to treatment. We do not bill or extend credit in anticipation of insurance reimbursement. If a balance remains after treatment, we will notify you. Any outstanding balance may be charged to the card on file or another approved payment method.

  • Balances over 7 days past due are subject to a $50 late fee
  • Accounts over 30 days past due may be sent to collections and may involve additional legal or credit consequences

Refunds and Account Credits

A credit balance may occur if:

  • Treatment is modified or completed at a lower cost
  • You decide not to proceed with planned treatment
  • Insurance pays the office directly

Once treatment is complete and your account is finalized, refunds will be issued for any remaining credit balance. Please note:

  • Refunds cannot be issued if there is an outstanding balance
  • Credit/debit card refunds incur a 3% fee due to processor fees passed on to us
  • Refunds to cards must be issued to the original card
  • If the original card is unavailable, a bank check will be issued
  • Cash payments may be refunded by cash or check

Please allow adequate time for processing, typically 2-3 weeks.

Delinquent Accounts

Unpaid balances requiring collection efforts may result in the responsible party being liable for legal fees, court costs, and associated expenses as permitted by law.
